Why do African Americans have exceptionally high rates of hypertension, diabetes, and obesity? Is it their genes? Their disease-prone culture? Their poor diets? Such racist explanations for racial inequalities in metabolic health have circulated in medical journals for decades. Blood Sugar analyzes and challenges the ways in which “metabolic syndrome” has become a major biomedical category that medical researchers have created to better understand the risks high blood pressure, blood sugar, body fat, and cholesterol pose to people. An estimated sixty million Americans are well on the way to being diagnosed with it, many of them belonging to people of color. Anthony Ryan Hatch argues that the syndrome represents another, very real crisis and that its advent signals a new form of “colorblind scientific racism”—a repackaging of race within biomedical and genomic research. Examining the cultural discussions and scientific practices that target human metabolism of prescription drugs and sugar by African Americans, he reveals how medical researchers who use metabolic syndrome to address racial inequalities in health have in effect reconstructed race as a fixed, biological, genetic feature of bodies—without incorporating social and economic inequalities into the equation. And just as the causes of metabolic syndrome are framed in racial terms, so are potential drug treatments and nutritional health interventions. The first sustained social and political inquiry of metabolic syndrome, this provocative and timely book is a crucial contribution to the emerging literature on race and medicine. It will engage those who seek to understand how unjust power relations shape population health inequalities and the production of medical knowledge and biotechnologies.

In today’s world, responsible biocitizenship has become a new way of belonging in society. Individuals are expected to make “responsible” medical choices, including the decision to be screened for genetic disease. Paradoxically, we have even come to see ourselves as having the right to be responsible vis-à-vis the proactive mitigation of genetic risk. At the same time, the concept of genetic disease has become a new and powerful way of defining the boundaries between human groups. Tay-Sachs, an autosomal recessive disorder, is a case in point—with origins in the period of Eastern European Jewish immigration to the United States and United Kingdom that spanned the late nineteenth and early twentieth centuries, it has a long and fraught history as a marker of Jewish racial difference. In Testing Fate, Shelley Z. Reuter asks: Can the biocitizen, especially one historically defined as a racialized and pathologized Other, be said to be exercising authentic, free choice in deciding whether to undertake genetic screening? Drawing on a range of historical and contemporary examples—doctors’ medical reports of Tay-Sachs since the first case was documented in 1881, the medical field’s construction of Tay-Sachs as a disease of Jewish immigrants, YouTube videos of children with Tay-Sachs that frame the disease as tragic disability avoidable through a simple genetic test, and medical malpractice suits since the test for the disease became available—Reuter shows that true agency in genetic decision-making can be exercised only from a place of cultural inclusion. Choice in this context is in fact a kind of unfreedom—a moral duty to act that is not really agency at all.

For nearly forty years, feminists and patient activists have argued that medicine is a deeply individualizing and depoliticizing institution. According to this view, medical practices are incidental to people’s transformation from patients to patient activists. The Biopolitics of Breast Cancer turns this understanding upside down. Maren Klawiter analyzes the evolution of the breast cancer movement to show the broad social impact of how diseases come to be medically managed and publicly administered. Examining surgical procedures, adjuvant therapies, early detection campaigns, and the rise in discourses of risk, Klawiter demonstrates that these practices created a change in the social relations-if not the mortality rate-of breast cancer that initially inhibited, but later enabled, collective action. Her research focuses on the emergence and development of new forms of activism that range from grassroots patient empowerment to environmental activism and corporate-funded breast cancer awareness. The Biopolitics of Breast Cancer opens a window onto a larger set of changes currently transforming medically advanced societies and ultimately challenges our understanding of the origins, politics, and future of the breast cancer movement. The Children and Families Act 2014 takes forward a range of Government commitments which are intended to improve services for key groups of vulnerable children (children in the adoption and care systems, those affected by decisions of the family courts and those with special educational needs and disabilities) and to support families in balancing home and work life, particularly when children are very young. It takes forward legislation that has been announced in a range of Government documents over the past 2 years. The Act contains measures intended to remove barriers to adoption; reform the family justice system and the special educational needs system and ensure that services place children and young people at the centre of decision making and support. It contains measures which relate to the welfare of children, including areas such as: child performance; protecting children and young people from tobacco and nicotine addiction; young carers and parent carers; |staying put| arrangements; supporting pupils at school with medical conditions; reform of children's homes; clarifying the Secretary of State's intervention powers; and free school lunches. The Act also contains measures which support wider changes to childcare; introduces a new system of shared parental leave following childbirth or adoption; and extends to all employees the right to request flexible working. Through its reforms to the functions and role of the Children's Commissioner, the Act is intended to ensure that children in England have a strong advocate for their rights.

From 1999 to 2009, The Northern Manhattan Community Voices Collaborative put Columbia University and its Medical Center in touch with surrounding community organizations and churches to facilitate access to primary care, nutritional improvement, and smoking cessation, and to broker innovative ways to access healthcare and other social services. This unlikely partnership and the relationships it forged reaffirms the wisdom of joining |town and gown| to improve a community's well-being. Staff members of participating organizations have coauthored this volume, which shares the successes, failures, and obstacles of implementing a vast community health program.
